If you have become disabled due to an accident or illness, you may be eligible for benefits from your disability income insurance policy.

To notify Northwestern Mutual of a disability or illness, complete the Preliminary Notice of Disability form listed on the Disability Income homepage, or call Northwestern Mutual at the number listed below, and give us your full name, policy number, and daytime phone number.

We will mail you a personalized claim kit within two business days of receiving your notification of disability. The kit contains forms and instructions you will need to file your claim.

If you have purchased the waiver of premium benefit on your life insurance or annuity and IRA policies, you need only send one notification to begin the claim process for both.
