You will find information and quick references concerning several aspects of your policy, including online and downloadable forms that can be used to make policy changes. Your Northwestern Mutual Financial Network Representative can also assist you in making policy changes or providing additional information.

Use this online request form to submit changes about payer information.
Enroll or Change Electronic Funds Transfer Online
Use this online request form to sign up for Electronic Funds Transfer (EFT) or to change your existing EFT.
Report of Insured's Death
Use this online form to inform Northwestern Mutual of the death of an Insured.
Life Benefits Request for Service
Use this online request form to submit questions concerning the settling of your insurance or annuity contract under a settlement option.
Universal Life Insurance Request for Service
Use this online request form to submit questions or comments regarding your Northwestern Mutual policy.

For Variable Universal Life policies, use this form to transfer existing assets and/or change payment allocation instructions.
